All participants are required to answer the following screening questions prior to participation.

(a) I confirm that I have not experienced symptoms related to COVID-19,

(b)  Neither I nor my household members have been diagnosed with COVID-19,

(c)  I do not believe I have been exposed to someone who has a suspected or confirmed case of COVID-19

(d) I have been following all CDC guidelines.

 I further acknowledge that I am responsible for my own health, and acknowledge that there are risks of contracting COVID-19 in any number of ways, and that Modern Skate Park and all of it’s affiliates cannot guarantee my health and well-being. 

I acknowledge that Modern Skate Park takes all necessary precautions (requiring masks, cleaning regularly, offering disinfecting hand soap, implementing signage encouraging 6 ft of separation, taking temperatures and other precautions).
